Form DS-11 is the application you use to get a U.S. passport when you can't renew by mail — first-time applicants, anyone under 16, people whose previous passport was lost, stolen, or damaged, and those whose most recent passport was issued more than 15 years ago or before age 16. It's the most common passport form precisely because every first passport goes through it. The form collects your full name, date and place of birth, Social Security number, contact details, parental information (for minors), emergency contact, and travel plans.

The one rule that trips people up: do not sign the DS-11 ahead of time. Unlike a renewal, the DS-11 must be submitted in person at a passport acceptance facility (a post office, library, or clerk of court), and you sign it in front of the acceptance agent who witnesses your oath. Who needs to use Form DS-11 instead of renewing?
