About DS-82

Form DS-82 is the passport renewal application you can complete and mail yourself — no in-person appointment required — if you meet the eligibility rules. You qualify to renew with DS-82 if your most recent passport is undamaged and you can submit it with the application, it was issued when you were 16 or older, it was issued within the last 15 years, and it was issued in your current name (or you can document your name change with a marriage certificate or court order). If any of those isn't true, you generally need to apply in person on Form DS-11 instead.
